We accept referrals from family physicians and specialists only. Download the form you need, complete it, and fax to 416-398-1516 — we’ll book the patient within two business days.
Injection / procedure referral
For patients already worked up and ready for a specific image-guided injection (epidural, facet, medial branch, RFA, nerve block, joint injection, PRP).
